Supportive Housing
Supportive Housing is permanent housing coupled with community support services that are collaboratively tailored by the recipient of services and support specialists. Wrap around services are mutually established with the consumer, including but not limited to case management, medication monitoring and management, wellness and recovery planning (WRAP), illness management and recovery groups (IMR), integrated dual diagnosis treatment (IDDT), case management, linkage to community resources and naturally occurring supports, and budgeting/financial literacy training services.
Residential Care & Services
Residential Care Services include 24-hour supervised supportive services in a residential setting; and semi-supervised support in an apartment setting, designed to provide flexible, consumer-centered skill training to promote independence, self sufficiency, and wellness. Self care/daily living education, medication monitoring and management, WRAP, IMR, IDDT, linkage to mainstream resources, case management / service procurement, and budgeting / financial literacy training services are offered, tailored to each individuals needs (15 beds in Supervised Residence, 1 Respite bed in Supervised Residence, and 9 beds in Semi- Supervised Residence Level).
